• Médis Provider Network
  You will have access to the largest and broadest providers network in Portugal, with over 6.000 doctors, 1.500 auxiliary diagnoses test facilities, 600 clinics and 90 hospitals.
  Regardless of the coverages and available capitals, all Médis Clients are given access to this network at the cost agreed between Médis and the providers.
  
  
• Médis Card
  Your Médis Card will give you access to all Healthcare Providers within Médis Network. With it, an in-network Healthcare Provider will be able to bill Médis directly (except for co-payments) for the services you use. Additionally, it will offer you unique advantages when using a provider within the Complementary Health Services network, as well as all other Médis Network Providers, even is your insurance does not cover for Dental or Outpatient healthcare services.
  
  
• Linha Médis
  It is a round the clock call-centre operated by especially skilled nurses who will provide healthcare orientation to customers.
  
  
• Médico Assistente Médis
  Médis believes that it is fundamental to have a doctor that knows one well and in who one can fully trust. With this in mind, Médis created Médico Assistente Médis, a personal primary care physician with at least 10 years of professional experience who is available, for all Médis Clients assigned to him, from 9 am to 9 pm, and afterwards through his mobile phone.
  Thus, as a Médis Client, you will be given the opportunity to choose your own Médico Assistente Médis.
  
  
• Médis Personalized Attendance at Hospitals
  At CUF Infanto Santo, CUF Descobertas Hosptials and Luz Hospital, you can count with a personalized attendance by a Médis team in an exclusive desk, providing you with all the necessary information and answering any doubt you may have.

Overtime health has evolved from a mere healing concept to a broader view where one sees one's health not only as the absence of illness, but as both physical and mental well being.

With this in mind, Médis as created a complementary discount network with centres that offer services somehow related to healthcare, such as Health Club, Optics, Aesthetic Centres, Psychology, Speech Therapy, among others.

• Second Opinions Network
  It is composed by international reference specialist doctors who help in the diagnosis and treatment of your condition, giving guidelines for the best treatment and the best centres to do so.
  Currently you can have access to Clínica Universitária de Navarra in Spain, which is especially skilled in the areas on Oncology and Paediatric, and to Johns Hopkins in the US.
  
  
• Médis Providers Network in Spain
  Médis Providers Network also includes Hospitals in Spain, that you can use in the same manner as if they were located in Portugal, that is, just by presenting your Médis Card. Among others, this network includes:

• Hospital Virgem de la Consolacion e Hospital de Molina, em Murcia;
• Povisa, em Vigo;
• Clínica Ntra. Sra. Del Rosario, em Toledo;
• Grupo Iberica de Diagnóstico Y Cirurgia e Clínica Recoletas, em Madrid.


• International Network
  Médis International Network includes some of the most recognized hospitals in the world, which are: Johns Hopkins in the US; Clínica Universitária de Navarra and Barcelona Centro Médico in Spain; Deutsches Herzzentrum Berlin in Germany.
  Depending on your Médis Plan, you are also covered by a Dread Diseases Coverage with a capital of 1 Million Euros. It will enable you, in face of a critical condition (cancer, neurosurgery, coronary artery bypass surgery, replacement of heart valves, organ transplant), to have access to the most prestigious physicians and hospitals around the world.
  
  The Dread Diseases Coverage also offers you the InterConsultation? service provided by Best Doctors Corporation. This service consist on, in face of a serious illness, a second opinion from the world best physicians in Best Doctors database, who will analyse your clinical history and, if necessary, request further test in order to reach an accurate diagnosis and, thus, to advise you and your doctor on the best suited treatments.

• Benefits Statement
  Periodically you will receive a statement with all the information regarding the services you used, comprising the amounts you paid, as well as those paid through your Médis Health Insurance;
  
  
• Médis Directory
  A book with the full list of Médis Network Providers, easily searchable, organized by Medical specialty and by city;
  
  
• Internet
  Has the most developed health Insurance website (www.medis.pt), providing you with information regarding Médis as well as a restricted area for you to get all information you may need about your own Médis plan.

• Pre-existing diseases or accidents occurred prior to the effective date of your Health Insurance;
  
• Professional diseases or accidents;
  
• Mental illnesses, unless stated otherwise in the Policy Wording for Psychiatry appointments. Also excluded are any Psychology treatments or appointments, as well as psychotherapy, hypnosis or sleep therapy;
  
• Treatments, surgery or any other medical procedures specifically performed to correct any genetic malformation or malfunction, unless stated otherwise in the Policy Wording for newly born babies, provided they are include in the policy from the moment of birth:
  
• Plastic surgery, unless it comes as a consequence of an accident or disease covered by the policy;
  
• Accidents occurred during or as consequence of a professional or dangerous sport;
  
• Check-ups;
  
• Haemodialysis.

• 60 days for outpatient (including dental);
  
• 90 days for inpatient;
  
• 6 months for Dread Diseases Coverage.
  
• There is 12 months waiting period for the following:
  
  • Sclerotherapy and/or surgical treatment of varicose veins;
    
  • Surgical treatment of a spinal disc hernia;
    
  • Haemorrhoidectomy or any haemorrhoids treatment;
    
  • Arthroscopy;
    
  • Septoplasty.
    
  • Tonsillectomy, adenoidectomy and myringotomy with our without use of ear tubes;
    
  • Rhinoplasty;
    
  • Benign skin lesions excision;
    
  • Benign skin lesions laser treatment;
    
  • Surgical apnea treatment;
    
  • Birth.

Option 1 and Option 2

• Age limit for subscription is 64 years old;
  
• Age limit for permanence in the insurance is 65 years old, though, if the insurance is subscribed before the age of 55, there will be no age limit.
  

Option 3

• Age limit for subscription is 64 years old;
  
• There is no age limit for permanence in the insurance.
  

There are 3 options for you to choose:

(4) Primary Care Appointments: General Practice, Internal Medicine, Paediatrics, Gynaecology, Obstetrics and Ophthalmology.

International coverage description (included in Option 3):

• Option 3 will provide you the access to internationally prestigious health providers.
  

(5) Médis will pay the full amount of those Health expenses that were previously authorized and, in inpatient, it will also pay the Insured Person and one companion for their accommodations and travel expenses by airplane (regular Class), train or car (only includes gas, tolls and accommodations for one night in the trip to the institution and another in the trip back).
(6) Deductible of 1.500 EUR per family per year.
